⚠ Safety & who should talk to a provider first

Nitrates — absolute: Nitroglycerin, isosorbide, or amyl nitrite (“poppers”) taken with tadalafil can drop blood pressure to a dangerous level. There is no dose that makes this combination safe. The same applies to riociguat (Adempas).

Heart and blood pressure: A heart attack, stroke, or unstable angina in the last six months, uncontrolled blood pressure, or unstable heart failure — talk to your provider first.

Alpha-blockers: Tamsulosin, doxazosin and similar prostate or blood-pressure medications can stack with ED medication. Not a hard stop, but dose and timing have to be planned — tell us everything you take. Because tadalafil stays in your system longer, interaction planning matters even more.

Eye and blood conditions: A history of NAION or sudden vision loss in one eye, retinitis pigmentosa, sickle cell disease, leukemia, or multiple myeloma need provider review first.

An erection lasting over four hours: Go to an emergency department. Priapism is a medical emergency, and waiting it out can cause permanent damage.

How long does tadalafil last?

Considerably longer than sildenafil — long enough that most patients stop planning around it. That extended window is the main reason people choose tadalafil, and also why interactions have to be reviewed carefully first.

Is this dose FDA-approved?

The compounded troche itself is not FDA-approved, but every strength we offer stays inside the FDA-approved as-needed tadalafil range (5–20 mg, max 20 mg/day). Some clinics sell 80–100 mg tadalafil troches — four to five times the daily limit, with no approval or safety data behind those numbers. We don’t stock them.

Do I need labs or an exam?

For ED, usually not — Florida Statute § 456.47 allows a licensed provider to evaluate and treat from a written history when that’s sufficient. If your answers raise something, we’ll ask for a call or labs before prescribing.

Tadalafil or sildenafil?
