Taking it: Let the troche dissolve against your cheek or under your tongue about 30–60 minutes before. Avoid heavy, fatty meals right before — they slow absorption.
Adjusting: If the effect is too strong or not enough, message your provider — the scored design means many adjustments don’t even need a new pack.

What to know before starting

⚠ Safety & who should talk to a provider first
Nitrates — absolute: Nitroglycerin, isosorbide, or amyl nitrite (“poppers”) taken with sildenafil can drop blood pressure to a dangerous level. There is no dose that makes this combination safe. The same applies to riociguat (Adempas).
Heart and blood pressure: A heart attack, stroke, or unstable angina in the last six months, uncontrolled blood pressure, or unstable heart failure — talk to your provider first.
Alpha-blockers: Tamsulosin, doxazosin and similar prostate or blood-pressure medications can stack with ED medication. Not a hard stop, but dose and timing have to be planned — tell us everything you take.
Eye and blood conditions: A history of NAION or sudden vision loss in one eye, retinitis pigmentosa, sickle cell disease, leukemia, or multiple myeloma need provider review first.
An erection lasting over four hours: Go to an emergency department. Priapism is a medical emergency, and waiting it out can cause permanent damage.

Do I need labs or an exam?
