These four are compounded medications, prepared by a licensed compounding pharmacy to our prescription. The FDA does not review compounded medications for safety or effectiveness. The active ingredients are FDA-approved drugs; what's compounded is the sublingual form.
A note on Complete. The sildenafil and tadalafil in it sit well under their approved daily limits. Oxytocin is different — it's an approved drug, but only as an injection, so there is no approved dose for a troche. We include it because some men notice a difference; the evidence behind it is thin and we won't pretend otherwise. If you'd rather skip it, Combination is the same two medicines without it.
Why a troche
It melts, you don't swallow it
It sits against your cheek or under your tongue for a few minutes. Part of the dose absorbs straight through the lining of your mouth.
No water, no audience
Nothing to wash it down with and nothing to explain. It goes in a pocket and nobody watches you take it.
Two medicines, one dose
Sildenafil and tadalafil can be pressed into a single troche. That combination has to be compounded — you can't buy it off a shelf.
Scored into quarters
The cross on the face isn't decoration. If the full dose is more than you need, it breaks cleanly into halves or quarters — your provider will say when.
Every ED dose stays inside the approved range
Sildenafil
The FDA-approved oral strengths are 25, 50 and 100 mg, with a maximum of 100 mg in a day. Our troches are 55, 45 and 30 mg.
Tadalafil
The FDA-approved strengths are 5, 10 and 20 mg as needed, with a maximum of 20 mg in a day. Ours are 11, 15 and 10 mg.
The combinations
In Combination and Complete, each ingredient stays well under its own daily limit — they aren't stacked on top of a full dose.
What we won't compound
Anything above the approved maximum. Some clinics sell tadalafil troches at 80 or 100 mg — four to five times the daily limit. There is no approval and no safety data behind those numbers, so we don't stock them.
Your provider may start you lower than the strengths listed here. The right dose is the smallest one that works.
Testosterone support — enclomiphene
It works upstream
Enclomiphene blocks estrogen signalling at the pituitary, which raises LH and FSH and tells your testicles to make more testosterone. You're producing it, not receiving it.
Fertility is preserved
Replacement testosterone shuts down your own production and lowers sperm count. Because enclomiphene works the other direction, it doesn't. That is the main reason to choose it.

Enclomiphene is not FDA-approved. An application was filed in 2015 and the FDA issued a Complete Response Letter citing the design of the clinical trials. It has not been approved since. It is compounded by a licensed pharmacy to our prescription, for one identified patient at a time, and the FDA does not review compounded medications for safety or effectiveness. Enclomiphene is one of the two isomers in clomiphene, an FDA-approved drug. If you would rather take an approved product, say so — clomiphene can be prescribed off-label instead, and we'll talk through the difference.
Bloodwork is required, not optional. Low testosterone is diagnosed from two early-morning blood draws on separate days, together with symptoms — not from symptoms alone and not from a single test. We order the labs and you pay the laboratory directly at cash-pay rates. We repeat them after the first 90 days and then periodically. If your levels are normal, we will tell you so and we won't sell you a prescription.

Testosterone is a Schedule III controlled substance. Prescribing it takes a separate DEA registration, additional controlled-substance training, a delegation filed with the Florida Department of Health, and a check of the state prescription database before every prescription. We are working through that, and we would rather say "not yet" than take shortcuts with a controlled drug.

The monitoring that comes with it
Two early-morning testosterone measurements before we start, plus LH, prolactin, and a complete blood count. PSA as well if you're over 40. Repeated on a schedule, not when you remember.
What it costs you besides money
Replacement testosterone suppresses your own production and lowers sperm count, sometimes lastingly. If you may want children, enclomiphene above is the better starting point and we will say so.
Who it isn't for
Men with untreated sleep apnea, a high hematocrit, prostate or breast cancer, or a partner currently trying to conceive. Also men whose testosterone is normal — low energy has many causes and this is only one of them.

When these medications aren't safe
Nitrates — ED medication
Nitroglycerin, isosorbide, or amyl nitrite — "poppers." Taken together with sildenafil or tadalafil, blood pressure can fall to a dangerous level. This one is absolute. There is no dose that makes it safe.
Riociguat — ED medication
Sold as Adempas, for pulmonary hypertension. Same interaction, same answer.
Recent cardiac events
A heart attack, stroke, or unstable angina in the last six months. Also uncontrolled blood pressure — too high or too low — and heart failure that isn't stable.
Eye and blood conditions
A history of NAION or sudden vision loss in one eye, retinitis pigmentosa, sickle cell disease, leukemia, or multiple myeloma.
Alpha-blockers
Tamsulosin, doxazosin and similar prostate or blood-pressure medications can stack with ED medication. Not a hard stop, but the dose and timing have to be planned. Tell us what you take.
An erection lasting over four hours
Go to an emergency department. Priapism is a medical emergency and waiting it out can cause permanent damage.
Liver disease — enclomiphene
Enclomiphene is cleared by the liver. Significant liver disease changes whether it's appropriate and at what dose.
Clots and vision — enclomiphene
A history of blood clots, stroke, or deep vein thrombosis. Stop and tell us immediately about blurred vision, flashes, or spots — visual changes are a known effect of this class and are a reason to stop.
Erectile dysfunction and low testosterone are sometimes the first sign of cardiovascular disease, diabetes, sleep apnea, or thyroid disease. If your history suggests that, we'll say so and order labs rather than only handing you a prescription.
Questions
Do I need labs or a physical exam?
For erectile dysfunction, usually not — Florida Statute § 456.47 allows a licensed provider to evaluate and treat from a written history when that evaluation is sufficient to diagnose and treat you. For enclomiphene, bloodwork is required and there is no version of this where we skip it. If your answers raise something that needs a live conversation, we'll ask for a call or a video visit before prescribing anything.
What does "compounded" mean?
A compounding pharmacy prepares the medication to our prescription rather than dispensing a mass-produced tablet. That's how you get a dose that dissolves under the tongue, or two molecules in one troche. The FDA does not review compounded medications for safety or effectiveness. For the ED medications, the active ingredients are FDA-approved drugs at strengths inside the approved range and only the sublingual form is compounded. Enclomiphene is different — the drug itself has never been approved. We say so plainly rather than burying it.
What's in Complete, and why does it cost more?
Sildenafil 30 mg and tadalafil 10 mg, the same two medicines as Combination at lower strengths, plus 100 IU of oxytocin. The oxytocin is what costs more to compound. It's an approved drug, but only as an injection — there is no approved dose for a troche, and the evidence for it in sexual function is thin. Some men prefer it. If that trade doesn't appeal to you, Combination is the better value and we'll say so.
Why does your troche cost more than a generic tablet?
Because it's a different product. A generic sildenafil tablet costs a pharmacy a few cents and is dispensed off a shelf. A troche is made to order for each prescription. If price is what matters most to you, take one of the other two options above — we'll send the FDA-approved tablet wherever you want and charge you nothing for the medication itself.
Can I use insurance?
Not for our care, and not for the compounded troches or capsules. If we send an FDA-approved tablet to your own pharmacy, you can use your insurance there like any other prescription. Some patients prefer not to, because a claim creates a record with their carrier.
How quickly can I have it?
Evaluations are usually reviewed within one business day. A prescription sent to your local pharmacy is often ready the same day it's approved. Shipped orders take a few days to arrive. Enclomiphene takes longer to start because we need your bloodwork back first. Nothing is prescribed until a provider has reviewed your history.
Is this a subscription, and can I cancel?
Yes to both. You'll see the exact amount and the exact date of every charge before you enter a card, and you can cancel at any time. If you cancel, you keep what you've already been sent.
What if it doesn't work?
Message your provider. For ED, dose, timing, food, and alcohol all change how these medications behave, and the fix is often one of those rather than a different drug. For enclomiphene, we look at your follow-up labs — if your testosterone hasn't moved, the answer isn't a higher dose, it's a different diagnosis.
When does testosterone replacement open?
We aren't putting a date on it, because it depends on registrations and filings that aren't entirely in our hands. Tell us you're interested and we'll contact you when it's ready. In the meantime, if you may want children, enclomiphene is likely the better option anyway.
Do you treat premature ejaculation?
Bring it up at your evaluation. It's common alongside erectile dysfunction and worth discussing, but it isn't sold from this page today.
