Membership is open to adults 18 and older located in Florida. One-time $100 enrollment fee. Spouse or partner joins the same plan at 25% off. Twelve-month membership, cancel any time with 30 days' notice. Member savings apply to weight management, wellness and vitamin injectables, hair loss, men's health and ED, and testosterone support — ask for the current price list.
Deliberately small
250 members. Then we close.
Same-day care only works if the panel stays small enough to deliver it. When membership fills, it closes — and members keep their place.

Acute illness
Colds, flu, COVID-19, sinusitis, sore throat, bronchitis, urinary tract infections, skin infections, pink eye, yeast infections, STIs.
Chronic conditions
Blood pressure, cholesterol, cardiovascular risk, type 2 diabetes, prediabetes, thyroid, asthma, mild COPD, allergies.
Everyday complaints
Headaches and migraines, reflux, nausea, IBS, back and neck pain, sprains, arthritis, tendonitis.
Skin
Acne, eczema, dermatitis, fungal infections and common rashes — reviewed from your photographs.
Hair loss
Pattern hair loss and thinning — evaluation, treatment options, and monitoring over time.
Men's health
Erectile dysfunction, prostate symptoms, and testosterone evaluation where it's clinically appropriate.
Weight management
Managed alongside your labs, blood pressure and everything else — with member savings on medication. Also available on its own.
Mental health
Anxiety and depression — evaluation, non-controlled medication management, and regular follow-up.
Sleep
Insomnia and sleep quality, plus screening for sleep apnea and a referral for testing when it's indicated.
Longevity
Advanced lab panels and biomarker tracking, focused on what actually moves long-term risk — metabolic health, cardiovascular risk, strength and sleep.

Annual review, vaccination and screening guidance, cardiovascular screening, lifestyle counseling, quitting smoking.

Notes and letters
Sick notes, return-to-work letters, FMLA and leave forms, accommodation and gym-clearance letters, travel letters for carrying injectables, and letters of medical necessity for HSA or FSA.
Referrals and coordination
Imaging ordered at cash-pay rates, specialist referrals with a real handoff note, review of results from other providers, and follow-up after an ER or hospital visit.
What we don't do
No opioids or stimulants
We do not prescribe or refill opioids, stimulants, benzodiazepines, or sleep medication, for any member, at any plan level. Testosterone is the only controlled medication we will prescribe, and only once that service opens.
Not emergency care
For a medical emergency call 911 or go to the nearest emergency department. We are not an emergency service.
No insurance billing
We don't bill insurance for our care. You may still use your own insurance for labs and at the pharmacy.
18 and older, Florida only
Membership is limited to adults aged 18 or older who are physically located in Florida. We do not enroll Medicare beneficiaries.
DonoMed Concierge is a direct primary care membership under section 624.27, Florida Statutes. It is not health insurance, does not qualify as minimum essential coverage under the Affordable Care Act, and is not a substitute for health insurance or for emergency care.
